The Synergy program targets clinically complex, chronically ill members who struggle with psychosocial issues that lead to avoidable utilization and disproportionate costs. Because of their complexity, these members require a multi-disciplinary approach and collaboration across the care community to positively impact health status, improve outcomes and facilitate change.

A critical aspect of Synergy Targeted Population Management™, Synergy Connect “turbocharges” existing resources – health management services, providers, community organizations – making them more effective in managing member risk and ensuring the right, quality care. Synergy Connect integrates the clinical approach and enables health plans to more successfully reach and empower members.

Synergy Connect goes above and beyond traditional program engagement strategies. Through partnerships with our health plan clients and the provider community, Synergy Connect aggressively seeks to reach members who are otherwise unreachable through customary channels.

Through Outreach and Care Coordination, Synergy Connect “plugs in” to the care community. While other programs operate at arm’s length and do not fully engage with the provider network or community-based organizations, Synergy Connect streamlines and integrates care management.

Synergy targets complex members, often enrolled, appropriately, in multiple care management programs, or none at all. With the goal of maximizing the clinical impact of the Synergy program, Care Coordination focuses on unifying clinical management support, ensuring quality care and organizing the collaborative efforts of health plan, provider and community resources available to each member. This is done in part by joint case-level reviews (Clinical Collaboration) along with customized referral processes and follow up to help the member receive the care they need.

Synergy Connect’s Outreach Coordination function serves to extend engagement and empowerment efforts by having a local presence in the communities that our clients serve. In addition to directly reaching out to difficult to engage members, on-the-ground Synergy representatives develop relationships with key influencers (i.e., health plan, providers, and community-based organizations) to create an environment that encourages active participation and ensures appropriate resources are made available to the member. This premium service leverages high-volume physician practices, community health centers, shelters and group homes, and local health plan programs to engage and empower members to better health.
