Case Study: Outreach Coordination

The following case study is a real example of the local outreach efforts made through Synergy Connect to engage members in the Synergy program. This example shows how our Outreach Coordinators build relationships with local provider practices, make them aware of the complementary benefits of Synergy and leverage practices to engage difficult to reach members. With specific targeting and provider support we were able to engage the member and help her on her way to greater self-management.

Maria is a 54 year old Hispanic female and a member of a Medicaid plan. She is single and lives with two daughters (17 and 20 years old). She struggles with Diabetes, elevated cholesterol, Arthritis, Obesity, and a gastrointestinal disorder. In addition to having several chronic conditions, she is highly stressed by one daughter’s illness (need of removal of an ovarian tumor) and another’s significant behavioral health issues. She is uncomfortable communicating with others and has minimal familial support, partly due to a recent conflict with her brother’s family. She is employed part time as a house cleaner. Maria was identified for the Synergy program based on a utilization pattern that included multiple inpatient stays and ER visits.

After multiple attempts to reach her through standard Synergy engagement outreach, Maria’s case is referred to Synergy Connect’s Outreach Coordination service. A key aspect of Outreach Coordination is the aggressive search for members who otherwise cannot be reached through customary channels. Outreach Coordination includes an analysis of local area provider practices with a high volume of Synergy members. The analysis indicates that several members of the health plan’s Synergy Targeted Population, including Maria, are patients of Dr. Eduardo Perez.
