Medicare / Medicaid
Health Integrated recognizes that Medicare and Medicaid plan members bring demanding health profiles that require higher-cost health services, often unnecessarily. These members also face unique psychological, social and economic challenges. In order to help Medicare/Medicaid patients be as healthy as possible, we offer Synergy Personal Health Management Program® (Synergy). Synergy addresses the unique characteristics of these populations, particularly the impact of psychological well-being and social support on physical health. Synergy delivers a set of integrated interventions that most effectively addresses each person's particular barriers to health.

Synergy consists of:

• A dynamic, consumer-focused engagement experience
• An integrated biopsychosocial approach that results in improved clinical and cost metrics

Synergy is flexible and customizable. Typically, we deliver Synergy in concert with your existing case and disease management programs. For the members that fit the Synergy profile, we apply demographic and psychographic communication strategies that incorporate multiple modes of appropriate interaction including direct mail, phone, text, email, and the Web. Synergy can also provide a more intensive level of engagement such as geographically located outreach coordinators suitable for select populations.

With individualized attention, Synergy stabilizes psychological issues, educates, and motivates members toward lasting health improvement. Synergy's personal Care Coaching is essential to helping individuals understand and adhere to their evidenced-based care plans, with full decision-support for more than 20 chronic medical conditions seamlessly integrated into our delivery platform. Synergy also continually monitors and intervenes on behalf of members, which reduces incidents of deterioration and decompensation.

Veterans Affairs / Department of Defense
Active military and veteran populations are particularly challenging because of the diversity of needs. Older veterans frequently have multiple chronic conditions and co-morbid psychosocial conditions, either diagnosed or undiagnosed. Younger active military or veterans may have psychosocial issues such as PTSD, anxiety or substance use disorders, which can insidiously aggravate chronic medical conditions and overall health status. Our Synergy Personal Health Management Program® (Synergy) is designed to emphasize self-efficacy through our integrated biopsychosocial approach, continual engagement, and customized support.

Synergy's direct-to-consumer engagement model includes mail, telephonic, and web-based communications along with local outreach for select populations. This allows individuals who may be reluctant or geographically unable to access traditional veterans or military health facilities to obtain personalized support. Our dynamic engagement model also pairs a highly credentialed and dedicated personal Care Coach with all individuals in the program. Synergy Care Coaches work with individuals to stabilize co-morbid psychological issues, set goals, educate and provide motivation along the continuum of care. As a result, we see decreased utilization and better adherence to evidenced-based care plans. Synergy also ensures long-term results by continually monitoring individuals and intervening at any point to achieve and sustain optimal health.
