Health Integrated Announces Success of Executive Leadership Summit: The Patient-Centered Medical Home
Tampa, FL – July 28, 2008 – Health Integrated, Inc., a Targeted Population health management company focused on helping health plans deliver truly integrated care management, announced the success of its most recent Executive Leadership Summit, at which key executives from health plans across the country discussed emerging challenges and opportunities for incorporating the Patient-Centered Medical Home (PCMH) into their chronic care management strategies.
Key executives from leading health plans, physician groups and consultants from
across the country, representing over 18 million covered lives, attended the Summit
to learn of the latest successful implementation strategies for executing Patient
Centered Medical Home programs for their membership.
The Medical Home concept is the latest evolution in improving overall care to chronically ill patients, based upon the conviction that the best quality of care is provided not in episodic, illness-oriented, complaint-based care—but through patient-centered, physician-guided, longitudinal care that encompasses and values both the art and science of medicine. The result is improved healthcare at a lower cost.

For the most recent Executive Leadership Summit, presenters examined the implications for physicians and group practices, health plans and patients of current pilots applying the Patient Centered Medical Home principles. Additional panel discussions incorporated feedback, input and questions from all attendees, thus adding to the overall benefit of the Summit.
Key discussion points focused on:
- Measuring and evaluating program performance
- Establishing reimbursement strategies that align incentives
- Integrating existing chronic care initiatives to avoid overlap and maximize impact
- Supporting physicians in providing expanded access and psychosocial interventions
- Identifying the right physicians and physician groups

At the request of several customers, Health Integrated announced at the Summit the establishment of a PCMH Benchmarking Initiative, to be conducted throughout calendar year 2009. This Benchmarking Initiative will provide health plans with a wide variety of comparative data points on critical program design, implementation and execution elements, as well as moderated best-practices sessions, aggregated data analysis and fully-developed content for RFP responses.
