Tampa, FL – October 26. On October 21, 2004, the Disease Management Association of
America (DMAA) presented Health Integrated, Inc. the award for "Best DM Program
Intervention for Co-Morbid Depression" at its 6th annual DMAA Leadership Forum in
Orlando, Florida. The Forum is recognized as one of the largest and most respected Disease
Management Conferences in the country. The award was presented by Dr. Donald Fetterolf,
Vice President and Senior Medical Director of Highmark, Inc., a Blue Cross Blue Shield
health plan based in Pittsburgh, PA.
Dr. Fetterolf praised Health Integrated for its "leadership, innovation and proven impact in
the field of depression disease management."

Health Integrated's Depression Disease Management Program begins with rigorous patient
identification, sophisticated assessment and comprehensive case stratification methodologies
targeting individuals who have been diagnosed with the disease, those not yet diagnosed and
even those at high risk of developing a depressive disorder. Clinicians, in concert with
program participants and their physicians, then develop a customized intervention plan
tailored to the participants' specific clinical needs and level of severity. They monitor
participant compliance and progress throughout the program, adjusting the intervention plan
as indicated.
Compared to control groups, participants in the program have shown positive outcomes
relative to:
- improvement in depressive symptoms, based on Hamilton Depression Scale scores
- decreased barriers to medication compliance
- use of outpatient services for both medical and depression-related visits
- Emergency Room utilization
- medical inpatient utilization
- overall medical expenditures
