Case Management a Key to Program Success

The conference brings together industry experts from across the nation to discuss trends and new approaches to providing high-quality case management services. Health Integrated held four presentations at the conference and participated in one panel discussion. The sessions focused on the success of integrating key care management services and included:

  • "The Emerging Medical Director Role with Complex Case Management" – This presentation discussed the different perceptions of case managers, providers and patients and provided a model of moving towards expanded medical director support. This support includes comorbidity interventions, reviewing complex cases, dealing with difficult clinicians, grand rounds for the care management team and pharmacy management.


  • "Harnessing the Power of Physician Group Visits in Complex Case Management" – Both the modern approach to telephonic complex case management and the newly developed physician group visit have been proven to significantly improve outcomes while lowering costs of care. The combination of these two approaches holds great promise to achieve synergistic benefits far beyond that of either approach alone.


  • "Can America's Seniors Get the Care They Need?" – In 2000, 13% of the population was 65 or older. By 2030 this is expected to increase to 20%. The fastest growing segment of the senior population is 85 and older. The presentation included a comprehensive approach to chronic condition management and depression management. With current "siloed" approaches being inadequate, it presented a model for bringing disease and case management together for all medical and behavioral support. This comprehensive approach incorporates identification, screening, assessment, stratification, interventions, medication/treatment compliance and ongoing re-assessment. It brings members, providers and other programs together to treat the whole patient. The program has demonstrated measurable success, both clinically and financially.


  • "Supporting the PCP in the Practice Setting with Depression Disease Management" – This presentation focused on a cohesive approach to working with depressed patients and providing the integration and tools to support the primary care physician (PCP) community. Programs must be able to incorporate PCP consultation and education, especially with primary care physicians prescribing 50-55% of the antidepressants prescribed in the U.S. When utilizing this approach, especially when combined with effective case management and other programs, patients will experience significant clinical improvement while providing a substantial return on investment for the payer.


  • "Issues and Trends of Comprehensive Health Insurance for High-Risk Individuals" – High risk pools have been established or are being created in 32 states and have average annual growth of 15 to 20%. The panel discussion included health insurance pool board members and described the trends and challenges faced by health insurance pools and included detail on the clinical and financial successes achieved by some of these pools in utilizing an integrated approach.
