By using our advanced, innovative technology and an inclusion strategy that we co-develop with our heath plan partner, we determine which population will provide the greatest opportunity for change and impact.

Sophisticated, Proprietary Member Identification

From a multitude of sources, including medical and behavioral claims, HRA, lab and pharmacy, we cleanse, analyze, and aggregate data to provide a view into the population mix. With proprietary biopsychosocial filters, sophisticated relational models, evidence-based algorithms and advanced predictive modeling, we determine near and long term risks in your membership and can drill down to evaluate each targeted member in a completely new way.

Customized Inclusion Definition

With this new view into the membership, Health Integrated works in consultation with the health plan to evaluate the unique clinical and financial opportunities not adequately addressed by their existing programs. For example, this member segment may include the diabetic with depression who is unable to follow her treatment plan or the obese, CHF patient who uses the ER as a proxy for primary care. Complex, multi-chronic populations like these are not typically well served with traditional condition management programs. From this careful analysis, we jointly determine the member segments to target for program inclusion.
