As targeted members are engaged in Synergy Targeted Population Management® the next step is to deploy interventions that seamlessly address the spectrum of issues that affect member health status—across both medical and psychosocial boundaries.
Medical and Psychosocial Interventions
Unlike any other company, Health Integrated looks at the person not the disease. We encourage each targeted member to engage with a personal, dedicated Synergy Care Coach, a clinical expert in both physical and psychosocial health. Working with their Care Coach, a member will set goals and get help to address not only their chronic medical condition such as diabetes, but also, and often more so, psychological or social challenges such as depression, substance abuse or lack of family support. Interventions may also be delivered via other modalities that might be better suited to the member’s comfort level at any given time.
For every individual we work with, Health Integrated can draw upon a multi-disciplinary team of nurses, physicians, psychiatrists, and educators to complement the core program content on an individualized basis, as needed.
Emphasis on Behavior Change
Synergy’s intervention model incorporates the most up-to-date science on behavior change, reflecting the importance of making lasting lifestyle changes to avoid unnecessary hospitalization as a result of otherwise manageable chronic conditions. The Synergy model incorporates a full range of disease-specific assessments, evidence-based guidelines and interventions designed to accelerate the members’ ability to improve treatment plan adherence, and ultimately to improve their health status.
Ongoing Monitoring
Synergy also ensures long-term results by continually monitoring individuals and intervening at any point to achieve and sustain optimal health. Our unique interventions help members achieve the clinical outcomes necessary to reach and maintain a high quality of life while helping health plans realize substantial savings.*
